Intake Workflow

On intake, have a gentle conversation with the patient about any animals at home.

• Do you have an animal at home?
• Is a family member or caretaker looking after it?
• Is there a power of attorney (POA)?In case the patient is unable to make a decision.
• Obtain consent to contact (phone numbers + consent form).If they have no help and animals are at home, ask whether they will sign a consent for us to find help — e.g. contacting an Animal Control Officer to transport the animal to a kennel, or anyone the patient consents to.

If resources are needed, use the directory. If the patient chooses to relinquish, use a consent form.

• Identify a kennel or shelter that can take the animal.
• Arrange transport with an Animal Control Officer or consented contact.
• If relinquishing care, complete the relinquishment consent form.A signed form removes barriers for animal control and police to take possession and ensures the animal is humanely transported to a humane society (or veterinarian if needed).

Connect caretakers with food and the right help for any animal.

• Share pet food pantries with extra supply for people in need.

• Provide any consent forms the patient needs.
